Our team of behavioral health specialists include:

  • Nurse care managers
  • Social Workers
  • Patients Coordinators

We help patients to cope with both their behavioral and physical needs. We offer many programs to help both children and adults find the care that is right for them. We also:

  • Support patients when they come home from the hospital.
  • Link patients to behavioral health specialists.
  • Make sure patients have the medicine they need.
  • Link patients to community resources.
  • Teach patients how to care for themselves.
  • Coordinate a patient’s care with their primary care doctor.
  • Help with substance abuse and alcoholism.
  • Support patients with developmental disabilities.
