Our Social Work Team links you to resources for social, emotional and financial help. This includes:

  • Transportation
  • Food
  • Furniture
  • Shelters for the homeless
  • HIV/AIDS
  • Domestic Violence
  • Abuse/Neglect
  • Assisted Living, Long Term Care
  • Veteran Services referrals
  • Immigrant/Refugee referrals
  • Health Education
  • Behavioral Health professionals

The CCPGM Social Workers are  limited in how they can help with:

Medical Equipment:
Social Workers will help with researching financial resources if  your medical equipment is not covered by Medicaid.

Housing
Social Workers will provide resources for housing and coach you on the process.  Social Workers are  not able to obtain housing for anyone. Housing is in extremely short supply in Mecklenburg County. You need to be very active in the process of finding housing.

Disability Applications
Social Workers provide information and contact numbers for the disability application process but cannot participate in filling out the application.

Legal Issues or Insurance Caims
Social Workers do not get directly involved, but can help or refer the patient to the legal resources.
